Process for making a complaint or compliment

  • On the day of your procedure you will be asked to fill out an electronic feedback form prior to discharge.

  • In addition you may ask for a feedback form at any time in your patient journey (from reception to discharge), or fill out and send through the form attached below.

  • The form will be received by the Nurse Manager who will call you to discuss complaints, and add the complaint/compliment to the agenda of the Management and MAC meetings.

  • Feedback will be given to the person who reported the complaint, outlining steps taken to correct errors and improve quality processes where appropriate.
